Quad Document Submission Form

Submit Your Document

Instructions: You will be sending us an email with your submission. Before you send the email, please ensure you have all the required pieces detailed below.   Thank you for your submission!

Before you send us anything, please ensure you have included ALL of the following:

  1. Full Name
  2. Email address where we can reach you with questions
  3. Document, including its Title (or Video, including Title)
  4. Your Author Bio (20-50 words)
  5. Your Author Photo

We will need your written submission in either Microsoft Word or Rich Text in order to format it properly for inclusion on the Quad.  Feel free to submit a PDF version in addition, if you have one, but without a Word Doc or Rich Text version, we will be unable to publish your article.

If your document is a video recording, then we will need it as link to a Zoom-based recording or in Vimeo format.
*IMPORTANT NOTE: Your submission means that you have read and agreed to the following Consent Statement – indicating you own the copyright and you accept the terms of publication. If you submit anything that equates to your legal consent and agreement.

AUTHOR CONSENT TO HOST AND PUBLISH AUTHOR WORK ON WEBSITE

As a pre-condition of including my work among the articles and written content which comprises the Coach Quad, I hereby grant my unconditional consent and permission for CoachQuad.com to host, publish and otherwise make available my submitted work and content on the website, blog, and syndicate through RSS feed.

While I retain sole and exclusive ownership of all copyright to my work, I acknowledge that you may host and publish my work on the CoachQuad.com site without compensation or remuneration to me and that you may alter the placement or location of my work, the manner in which it appears, and the duration of its appearance within CoachQuad.com. If I determine at some time in the future that I wish to have my work removed, I agree to provide CoachQuad.com with prior written notice of my request to have my work deleted or removed and ninety (90) days for CoachQuad.com to comply with such request.

I hereby attest that I own the copyright for the above-referenced item and; thus; am duly authorized to grant any permissions described in this form. Note: if an item is copyrighted, only the individual who holds the rights may submit their item for review. If you are not the copyright holder for this item, please direct that individual to this site so that they may complete this form and have their item reviewed.

By checking the box below, I confirm my consent to the above and that I am authorized to give this consent. If I fail to check the box granting consent, my work will not be included within the content of the CoachQuad.com.
